members to evaluate collective perceptions of Overall
Organizational Health, the Board Governance Role, and
Board Practices and Effectiveness. The survey also provides
opportunities for self-assessment of commitment and effectiveness as
a Board member and short narrative responses about Board strengths
and opportunities for improvement. Questions from the sample
Board Assessment Survey below can be tailored for your

several report formats, including individual reports showing self
compared with composite results.
